PRE-EMPLOYMENT DISCLOSURE STATEMENT

As part of the application process, we ask you to answer a series of questions relevant to the role and our legal obligations under New Zealand law. Before continuing, please read and consent to the following.

 

Health and Safety Commitment

Our organisation is committed to providing a safe and healthy working environment, in accordance with the Health and Safety at Work Act 2015. To support this, we must ensure that employees can safely perform the inherent requirements of their roles.

 

Disclosure of Health Information

You are asked to disclose any physical, medical, or psychological conditions—past or present—that may affect your ability to safely carry out the duties of the role you are applying for. Failure to disclose relevant health information may:

  • Limit our ability to assess your suitability for the role.
  • Affect your eligibility for support or compensation under the Accident Compensation Act 2001 if a condition is aggravated or worsened during employment.
  • Impact our ability to meet our legal obligations to provide a safe workplace.

 

Privacy and Consent

All personal information collected during this process will be handled in accordance with the Privacy Act 2020. This includes:

  • Collecting only information relevant to the role.
  • Storing your information securely.
  • Disclosing it only to parties directly involved in the recruitment process (e.g. referees, occupational health providers), and only with your consent.

 

You have the right to:

  • Access and correct your personal information.
  • Withdraw your application at any time.
  • Request that your information not be retained following an unsuccessful application.
